Our staff is committed to providing on-site, comprehensive and cost-effective services. We’re able to provide services in a variety of locations, including in the client’s home, at school, or in another community setting to help them reach their specific treatment plan goals. However, we do not provide transportation for our clients.

Twin-Cities-Therapy-Clinic

Therapy

  • ACP provides individual and family therapy.  Our staff consists of highly qualified mental health practitioners, licensure track clinicians, and licensed providers.  We strongly recommend a client be receiving active therapy services (in home or in clinic) prior to utilizing additional services such as ARMHS or CTSS. In home or community-based therapy can support a client in meeting their treatment goals in an environment that allows the client to feel comfortable and secure. We can offer a range of treatments that may help with mental health problems, emotional challenges, and functional impairments.

Children’s Therapeutic Services And Supports (CTSS)

  • CTSS is a mental health service to support children and their families to improve functioning in the home, at school, and within the community.
  • CTSS services include individual therapy, family therapy, individual skills training, and family skills training. Therapy is required to receive individual/family skills and offer collaborative intervention.  
  • CTSS services are available to those under age 18 who have been diagnosed with a mental illness, meet medical necessity by determining that the child has lost behavioral skills or failed to develop behavioral skills compared to others of similar age as a result of the child’s diagnosed mental health disorder. In addition, the child must have insurance coverage of Medical Assistance (MA) or Pre-Paid Medical Assistance Plans.

School-Based Services

  • Through a grant from the MN Department of Human Services, our staff is able to provide services to students in their home, in the community (“school-linked” program), or in the school itself (“co-located” program). All services are conducted by qualified mental health professionals and practitioners.
  • ACP offers services to students in Dakota County partner schools regardless of insurance coverage. We will work directly with the insurance plan on your behalf to answer any questions.
  • School-linked and co-located services include individual therapy, family therapy, individual skills training, and family skills training. Our dedicated providers work closely with the child’s family and school to provide collaborative care.

FAQ's

Adult Rehabilitation Mental Health Services (ARMHS)

  • ARMHS is a mental health service intended to support, assist, and educate clients in developing and strengthening skills to manage demands of community-based (independent) living.
  • ARMHS services are available to individuals age 18 years or older who have been diagnosed with a mental illness, meet medical necessity requirements, and have insurance coverage of Medical Assistance (MA) or Pre-Paid Medical Assistance Plans (PMAP).

Early Intervention (Children Ages 0-5)

  • Through a grant from Dakota County, our staff is able to provide services to children ages 0-5 in their home and/or community as indicated. All services are conducted by qualified mental health professionals and practitioners.
  • ACP offers these services to children in Dakota County regardless of insurance coverage. We will work directly with the insurance plan on your behalf to answer any questions. We share this grant with one other mental health agency in Dakota County. Our referral coordinator can guide you if we are not the appropriate mental health agency based on your location.
  • We utilize the comprehensive Diagnostic Assessment (DA) for assessing infants, toddlers and preschool children. This tool classifies mental health and developmental disorders in children from birth through five years old, which includes consideration of family, culture, and community.
  • This expansive Diagnostic Assessment then leads to further mental health intervention with our Community Based Therapy (and CTSS if qualified/needed) for your child/family. This can also help to establish whether a child qualifies for Early Childhood Intervention services (i.e. Help Me Grow).
  • If a child is ages 0-5, a Comprehensive Diagnostic Assessment would occur over 3 sessions and ideally at both home and a setting to observe play (i.e school/daycare/ECFE).

Culturally Specific ARMHS/CTSS

ACP is proud to partner with several agencies to provide culturally specific ARMHS and CTSS to the immigrant communities (including but not limited to those of Somali and East African descent) in the Twin Cities metro. We are often able to pair clients with a Mental Health Practitioner that speaks the
same language and shares similar cultural experiences.

Referrals can be made directly through ACP via phone at 612-455-8643 or online at acp-mn.com. ACP will screen referrals, complete an intake and assessment, and assign to a partner agency. ACP remains involved with these clients to provide ongoing clinical support to the partner agencies and their staff. Individual and family therapy is also available as needed.
